There are new versions of the Child Care Program Record Check Authorization Forms available on the CDD website (CC-Criminal-Record-Check, CC-Multi-Site-Record-Check). The new forms include a section on the second (2nd) page regarding the use of fingerprints to update FBI records ("Applicant Notification of Procedures to Update an FBI Record"), as required under 28 CFR 16.34. CDD will not be able to accept any Record Check Authorizations using the old form that is missing this section.
An updated version of the Fingerprint Authorization Certificate (FAC) Form is now available on the CDD website (CC-Fingerprint-Authorization-Certificate). The revised version has been updated to reflect the current director of child care licensing’s signature (Beth Maurer). The Vermont Crime Information Center (VCIC) will not release fingerprint results to CDD if the individual is using the previous version of the form without this signature; using previous versions will delay VCIC’s review process.
