Health and Safety Monitoring Checklists

The licensing team worked with federal technical assistance for over 16 months to develop comprehensive health and safety monitoring checklists for afterschool child care programs, center based child care and preschool programs, and family child care homes.

The Child Care Development Fund (CCDF) contains federal requirements that all states must ensure they are monitoring 11 core health and safety topics during every unannounced, annual compliance site visit. Our team worked diligently to develop these checklists, which you can now find our forms for providers webpage and linked below:

As a provider you may have noticed that annual site visits take longer, involve different questions than previous years, and require directors (or their designee) to sign the cover page of the licensing checklists.  

We appreciate your patience as we continue the rollout of these checklists in the field and want to assure you that:

  • The request of you to sign the cover pages is an acknowledgement that a post-visit debrief happened same day or is scheduled at an agreed upon future date.

  • Your signature does not equate to agreement of the findings. Should there be any violations issued, you still have appeal rights.
