To apply for services from OCS, you must complete and submit the Application for Child Support Services.

1. Read the Application Instructions. Print and keep a copy for your records.
    

2. Complete and sign the Application Form. Gather copies of all required documents (see the back of the instructions for a checklist).
    

3. Submit your application form along with the required documents to:

   103 South Main Street
   Waterbury, VT  05671-1901
