These are services that are available for children birth to age 3 years that have a disability or are at risk for a developmental delay.

Submitting Medical Information on Behalf of Patients

If you are involved in direct patient care, you may receive requests from the Vermont Office of Disability Services to submit medical information on behalf of your patients.

Vermont Office of Disability Determination Services (DDS)

DDS determines the eligibility of Vermonters who apply for disability benefits under the Social Security Disability Insurance (SSDI), Supplemental Security Income (SSI), and Medicaid.
